In most public profile situations the media will paint the fastest picture it can. It has to. Your problem is that the picture will be often based on the witnesses and participants who were least involved. It will also emphasise the aspects that "sell news"

The people who were involved with the incident (even at arms length) should not be involved in the investigation and even those with the best intentions can be your worst enemies.

Our role is to ensure the flow of public information is factual, you are protected from misinformation or inconsistent information and that the cause of the incident is acurately determined so that remedial action, be it systems change, equipment repair and training is swift and you are operational again.
